Information About the Form
To prepare Your application before meeting we need all Your personal details like: name, details from Your passport (serial number, expiry date, place of birth, authority), email address, contact phone number, postal address, Polish pesel number details (if you have it).
How it works? All details which You provide will be send to us by encrypted E-mail. Uploaded files will be send to our server. When we generate a documents, You will receive a confirmation to provided E-mail address. From that moment all Your provided details were permanently deleted from our mailbox and server. We store that details only to generate a documents for You.
HOW TO FILL THE FORM?
- Add file with scan of Your passport – we need all Your personal details to generate a documents. Only jpg, jpeg, pdf, doc, docx files are accepted.
- Enter Your Polish PESEL Number if You have it.
- Add file with scan of decision about issue of a Polish PESEL number. Only jpg, jpeg, pdf, doc, docx files are accepted.
- Enter Your E-mail address.
- Enter Your contact phone number.
- Enter Your postal address.
- Send Application.
- When documents will be ready, You will receive an email with confirmation about it from Certum by Asseco
